Female Hair Transplant?

Hi Dr. Mohebi,
I  am a woman and undergone three hair transplants.  The first two were in 2002 and 2003 for 1,200 grafts each. Then I decided to get another one for more fullness.  I am 11 months into my 3rd hair transplant with another doctor. When I review my images one year after my first hair transplant of 1,200 grafts it seems like I had more hair then than now after additional 2,600 grafts.I am very disappointed at the loss of 2,600 irreplaceable grafts and need to see if I am a good candidate for a fourth hair transplant surgery.  I really appreciate your input in this.

First of all you need to be seen and evaluated by a good hair transplant surgeon  It is important to know what is the condition of your scalp health and hair.  2,600 is a reasonable number of grafts to make a significant improvement in most people.  At times the full result of transplanted hair might be seen with some delay.  That is especially the case when you have some good amount of hair from before.  A completely bald person can see obvious growth of hair in about six months, but the following hair transplants may need more time to show their full results.  This could be assessed with a miniaturization study with a microscope and we can understand whether you have any new hair growing in the newly transplanted areas.

Hair Transplant Donor Wound Scar Revision

Hello Dr. Mohebi,

I had a previous hair transplant which overall was not too bad. It was about ten years ago. I want to know what could be done with the scar at the back of my head. I do some on screen work and now I wear my hair shorter or would like to. Can the scar from my FUT be improved? Another showbiz friend said you are known to be excellent at this. Is there any hope of doing that?

Great question John!

Scar revision is one of the areas I really enjoy and have the honor of being able to help patients. Scarring like balding really negatively impacts people’s lives. It is rewarding to be able to help in this way. In our hair restoration practice we take extraordinary care to minimize the scar of an FUT hair transplant surgery. In fact I was able to develop and invention that is gaining widespread use in hair restoration. It is called the Laxometer. By allowing the surgeon to more accurately measure scalp laxity he or she is able to better plan the minimization to the best of ability of the donor wound.

Along with special wound closure techniques there are other ways in which modern hair transplant yields more natural, proven and permanent results by minimizing the scar.

1. Simple Scar Revision: This method can be used if the scalp scar is wide due to initial technical problem during wound closure.

2. Scar Revision and Trichophytic closure: This method allows hair to grow inside the scar and minimize the contrast between scar and surrounding areas of the scalp.

3. Hair transplantation inside the scar: In this method, we harvest hair from other areas of the scalp and transplant inside scar to blend in the scar: Some people may need more than one surgery, but the result is usually great. We can utilize FUE hair transplant procedure in this method. The primary benefit of FUE is that follicles are removed individually and can be then placed into the scar area. So new scarring does not occur during the revisioning.

4. Tattooing the scar: By tattooing stubble on scar the visibility of the scar can be significantly improved through the balancing of the color or reducing the contrast in color between the scar tissue and the surrounding scalp area.

Again the final method will be discussed during our consultation and patients may need a combination of these techniques to provide the best result.

Hair Transplant Technician Skills and Quality Control

It seems to me that successful outcomes can, to a substantial degree, rely on the technicians handling the hair follicles. How do you measure your technicians skills?

A:

We have a great quality control system at US Hair Restoration in which every technician is monitored during cutting and placing.  We also have surveillance cameras in the OR to record the entire procedure for quality control.  We hire only highly qualified hair transplant technicians.  We still need to provide some special training to assure they can function according to US Hair Restoration standards.  We hold regular educational meeting for hair transplant techs to keep them all up to date on current procedures as well as US Hair Restoration quality expectations.

  • If a client has already had a transplant with another hair transplant doctor and is disappointed with the hair restoration results, can the procedure be redone?

Yes, hair transplant repairs are a good portion of our hair transplant surgeries performed in our Southern California offices (located in Beverly Hills, Encino and Orange County). Many people may have unnatural results from previous hair transplants due to the older techniques such as plug surgery, but some may even have problems with recent hair restoration procedures due to technical issues or lack of proper judgment on the part of the hair transplant surgeon. In most situations, the repair is easy and can be done in one or two sessions.

  • Is there a minimum age for hair transplant surgery?

It was not unusual that a Hair transplant surgeon discourage a person younger than 24 for a hair restoration surgery.  However, nowadays microscopic techniques being used to evaluate the miniaturization of susceptible hair can assist surgeons in predicting the final pattern of hair loss. By being able to predict each patient’ s final balding stages, we are able to see where the pattern of baldness is heading and based on that recommend hair transplant surgery at an earlier age.

  • Won’ t my bare scalp be showing in the back of my head once the donor hair is taken?

If you do not plan on shaving your hair after your hair transplant surgery, your donor wound will never be visible. However, if you plan on shaving your head for any reason, you may choose to have a FUE or Follicular Unit Extraction procedure done rather than the general FUT procedure through removing a strip from the donor area. FUE transplant eliminates the visibility of a linear scar on the back of your head and is the best option for patients who plan on shaving their head or keeping their hair very short in the future.

  • Do you provide free consultations?

Yes, the initial consultation with me (Dr. Parsa Mohebi) is always free in any of our Southern California hair transplant offices in Beverly Hills, Encino and Orange County and San Diego.

  • What type of information should be brought to the consultation?

I come from china ,I would like to consult your organization a few questions:

In your experience,usually Asians(chinese people) the normal growth of the number of roots Per cm² of hair? Head at the top and post-occipital whether there are different?
using FUE technology,on one operation ,After the occipital Site  get the maxinum number of FU (Asians including 2-3 days operation) at present,how much FU can be planted per square centimeter on Planting area? Between the scope of? and the percentage with normal?

Here are the answers in the order they were received:

The number of hair follicles

The number of hair follicles in non-balding areas of scalp like occipital areas have been studied.  A range of 80 – 120 follicular units (FUs) per centimeter were reported. The density of follicular units vary in different locations of the scalp. Also, the number of hairs per follicular unit vary in different areas and with different races. For example, Asians usually have less number of hair follicles per cm², so even with similar number of FUs per cm², the density of their donor area is still less than Caucasians. To calculate the number of hair per cm², you have to average the number of hairs per FU and multiply it by the number of FUs per cm².

Difference of hair on top and back

The shaft of hair may or may not be different in different areas of the scalp. Obviously, if you are balding, you see more miniaturization and less terminal hair, which makes the appearance of the hair less dense on the area. However, without baldness, hair quality would be the same in back and top of the head.

FUE and number of grafts

FUE (Follicular Unit Extraction) is more labor intense and requires more time for harvesting the grafts. For that reason, FUE procedures are generally smaller in terms of the number of grafts per session. What we do at US Hair Restoration is up to 1,500 grafts per day on average, for 8 to 10 hours work in a FUE procedure. The number may occasionally go up to 2,000 grafts, too, but not everyone can expect to get that number with FUE.

Density of transplanted hair

There are many research projects on this topic. The number can be as high as normal density in some circumstances. However, practically we can make up to 35%-40% of the normal density in one session. This is a good number if you consider that the density of hair on frontal areas could be about half of the hair on the donor area in some patients, so 35%-40% is not that far from the normal density in those patients.

Medical Hair Restoration – Effective or Not

The effectiveness of medical hair restoration alone with hair restoration medical options that are currently available such as finasteride or Rogaine has always been questioned.  Medical hair restoration without the surgical option (hair transplant), might work for some people, but should not be recommended to all as a cure-all solution, as you may find in some advertisements.

The available FDA-approved medications for hair loss prevention are finasteride and minoxidil.  Although both are effective, they may have different indications for male and female patterned baldness. They might not be considered a complete treatment for male patterned baldness, as pharmaceuticals may try to show.

Medical hair restoration should be seen as a preventative measure which should be considered, along with the surgical options, as part of a master plan. Most people are prone to shock loss that may happen after a hair transplant surgery and they need to be on preventative measures such as finasteride at least a few months around the time of surgery. Shock loss after a hair transplant used to be one of the biggest problems that hair transplant patients had up until the last few years. Now, most hair transplant patients go on finasteride after a hair transplant surgery.
